The U.S. Army is investigating serious safety concerns involving its Apache attack helicopter fleet after a string of major crashes and the discovery of a potentially dangerous transmission problem affecting some aircraft.
Five major Apache crashes have occurred since last October, putting fiscal year 2026 on pace to become the second-worst year for Apache flight accidents since the AH-64E model entered service in 2011.
The growing concerns involve more than military hardware. They raise important questions about U.S. military readiness, taxpayer-funded defense equipment and the safety of American soldiers who operate one of the Army’s most important combat helicopters.
Deadly Apache Crash Raises New Safety Concerns
Concerns intensified after an Apache helicopter crashed in Texas earlier this month, killing two U.S. soldiers and starting a wildfire.
Following the deadly accident, the Army restricted Apache pilots from conducting routine training flights while officials examined the situation.
The crash came during an already troubling period for the Apache fleet.
According to data reported by CNN, the Army’s major Apache accident rate for fiscal year 2026 stands at 6.24 mishaps per 100,000 flight hours.
For comparison, the rate reached 7.02 major mishaps per 100,000 flight hours in 2024.
That means 2026 is currently approaching one of the highest major accident rates recorded since the AH-64E was introduced.
What Is Causing the Apache Crashes?
Investigators do not appear to have identified a single explanation for the recent accidents.
A July Army aviation safety publication reportedly found that human error was the leading factor in just over half of this year’s Apache mishaps, including incidents that were not classified as major accidents.
It remains unclear how many accidents were caused by mechanical or equipment problems.
That distinction is important because the Army is separately investigating a potentially serious transmission issue involving some AH-64E helicopters.
The Apache is a highly sophisticated aircraft, and a malfunction involving critical components can have serious consequences for pilots and crew members.

An Army spokesman said restrictions surrounding technical aviation information prevent the service from disclosing whether the crashes are connected to the transmission issue.

Dozens of Apache Transmissions Flagged
The transmission problem was under investigation before the deadly Texas accident.
Defense One reported in May that approximately 70 potentially faulty transmissions had been identified. Army units were subsequently instructed to stop flying affected Apache helicopters until the necessary work could be performed.
The problem grew in July when another 10 suspected transmissions were reportedly added to the no-fly list.
Altogether, the affected equipment reportedly involved approximately 10% of the Army’s attack helicopter fleet.
The Army has said that Apaches affected by the transmission issue remain grounded.
Keeping potentially affected helicopters out of the air protects pilots and crews while investigators and maintenance personnel work to resolve the problem. However, grounding aircraft can also create challenges for training and military readiness.
Apache Problems Put Military Readiness in Focus
The Apache is not an ordinary helicopter.
The AH-64 is one of America’s most recognizable attack aircraft and is designed to provide heavily armed battlefield support for U.S. forces. Apache helicopters have served in American military operations around the world for decades.

The defense contractor told CNN that it is supporting the Army’s investigation as officials work to ensure affected aircraft are safe and operationally ready.
Several major questions remain unanswered.
Investigators still must determine exactly what is driving the unusually high accident rate, whether equipment failures contributed to particular crashes and how quickly helicopters affected by the transmission problem can safely return to service.
What Happens Next With the Army’s Apache Fleet?
The Army’s investigation will be closely watched as officials determine the scope of the transmission problem and evaluate the circumstances surrounding recent Apache crashes.
